Update Khoj Plugin Install Instructions. Rename main Readme to README

Browse Source 
Khoj plugin page from within Obsidian isn't recognized. Seems like it
needs an uppercase readme file only. So it doesn't show the Khoj
readme from within Obsidian itself.
